Passport Applications That Require a Personal Appearance
You MUST apply for a passport in person if:
1. Your previous passport was issued when you were under 16, or your (or your child's) current passport was issued for a 5- year validity period (for minors, please see below for special requirements).
2. Your previous U.S. passport was lost, stolen, or mutilated (please see below for special requirements).
3. You are applying for a U.S. passport for the first time.
4. All new passports will be returned by mail using a Chronopost tracking system. Please enclose a self-addressed Chronopost envelope and note its tracking number.
OR
5. Your name has changed since your passport was issued and you do not have a legal document formally changing your name.
If you must apply in person, please submit the following when coming to the U.S. Consulate for your appointment:
1. A completed form DS-11. Please do not sign this form.
2. Your most recent passport.
3. Two (2) identical, recent, color photographs in correct format (see list of photographers).
4. All new passports will be returned by mail using a Chronopost tracking system. Please enclose a self-addressed Chronopost envelope and note its tracking number.
5. You must present proof of U.S. citizenship with any one of the following:
- Original previous U.S. Passport (mutilated, altered, or damaged passports are not acceptable as proof of U.S. citizenship.)
- Original certified U.S. birth certificate issued by the city, county or state.
NOTE: A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office, which must be within one year of your birth. - Original Consular Report of Birth Abroad.
- Original Naturalization Certificate.
- Original Certificate of Citizenship.
6. You must present proof of identity with any one of the following, if you are recognizable:
- Original previous U.S. Passport (mutilated, altered, or damaged passports are not acceptable as proof of U.S. citizenship.)
- Original Naturalization Certificate.
- Original Certificate of Citizenship.
- Original current, valid, driver's license; government ID: city, state or federal; military ID: military dependents; resident card.
- NOTE: Your Social Security Card does NOT prove your identity.
7. Payment (see fee schedule) in Euros by mandat-cash available for purchase at the French Post Office or by credit card for payment in Euros or U.S. dollars.
We do not accept cash or personal checks.
Special requirement for Minors under Age:
Change in Requirements for Passports for Minors
As of February 1, 2008, the two-parent consent requirement for all passport services will be extended to include all children under age 16. This requirement was in effect for all children under age 14. Either both parents must be present to provide signatures at the time of passport application, or in the event one parent cannot attend, they must submit a notarized statement of consent and identification. Alternately, the applying parent may present a death certificate or sole custody order for the child.
All seven conditions listed above apply AND:
- Original certified U.S. birth certificate or Consular Report of Birth Abroad.
Lost, Stolen or mutilated Passports
All seven conditions listed above apply AND:
- You must submit a police report substantiating loss or theft AND all available identification such as: birth certificate, driver's license, photocopy of the missing passport, or expired U.S. passport.
- If you have no current identification with you, you should bring along an American friend or relative with his or her passport to vouch for you.
- All applications for children under 16 require both parents' or legal guardians' presence and consent. If applying for a minor under 16, you must also submit an original of the child's French or U.S. birth certificate and his/her Consular Report of Birth Abroad if applicable.
